renewal of visa from outside India

Hi
I have to renew Tier 2 dependant visa for my spouse and kids. Their visa expired on July 15th 2019 (was from 15/07/2018-15/07/2019) and going to apply for the renewal on August 15th 2019 . Do they require TB test and police clearance as they were one month outside UK ?
How does this going to affect indefinite leave to remain after 5 years ?
