IHS refunded before decision

I hope everyone doing well. Someone could you please advise me please. I've applied uk spouse visa.
online application submitted 1st may
biometric taken 6th may
documents lodge 15th may
10th of August UKVI refunded IHS fee I presume my application has been refused. I've not received any refusal letter yet it's nearly 4weeks.
Anyone have ideas what's going on?
Why they taking long give to me refusal/decision?
