Tier 2 visa extension with 8 months until ILR

Hello everyone

I am wondering if someone has had a similar experience.

My current Tier 2 visa ends on 15/8/2021. From then I will have 8 months left until I can apply for ILR.

From what I can see, my next contract will be for 5 years (as its a specialty training which lasts for 5 years).

I suppose they will not grant me a visa for another five years as its gonna be 9 years in total on Tier 2.

However my quesiton is will I need to pay fees like health surchage etc for 5 years, or I just need to state in my cover letter that it is only 8 months left until my ILR and they will issue me a visa for 1 year? How does it work?
