Set O - Evidence of your absence from the UK

I'm applying for my IRL from a Tier 2 (General Visa) and I'm a bit stumped.

For the question with Evidence of your absence from the UK and Crown Dependencies, it says that `Evidence of all work-related absences (including paid annual leave) is required from those applying under....`

Does this mean that for every holiday I took, I will need a letter saying that I was definitely granted days off during those days?

Because the company I work for is multinational, I have quite a few trips where I travelled to work in an office in another country or going for conferences, will these have to be covered/mentioned as separate 'types' of absences in the letter? i.e. not counted as days off?

My 5 years is also up on the 8th of October but I have a trip to Barcelona on the 7th-11th; will it be a problem if I send my application after the 11th?

When did you first enter the UK? (Required)
This is the date you entered the UK when the period of stay you are basing this application on started.

I entered 08 October 2014 (date I got my first tier 2 general visa) but I was actually in the UK since October 2011 as a student, which date should go into this box? I entered the 2014 date because it says the period of stay that I am basing this application on.
