ILR Stamp

Can anyone please tell me what the current policy is on ILR stamps. Can an ILR stamp be placed by an IO at a port in a new passport or does this now have to be sent to the Border Agency for them to affix a "Visa" style sticker or whatever inside a passport. How do regular passengers get around this?
