Letter from gp and school

I would be needing a letter from both the GP and the school authorities and I am not sure how the letter should be worded or what its needed to be written.

I am about to apply for my 13 year old son visa which I believe is through the FLR FP form.

He already completed 7 years in UK

Appreciate if you can help on it
