Tier 2 Dependant visa Extension

Hey there I am planning to apply for tier 2 visa Extension for my wife and kid. I have a few questions for the same.

1) I have a Tier 2 General visa till 2023. Their current visa expires on 13/03/2020. I am planning to apply early January probably around 06/01/2020, as it leaves around 8weeks processing time mentioned in the website. My worry is with the recent trend of difficult in securing appointment with UKVCAS, will their application result extend beyond their current visa expiry date? Do they calculate 8 weeks from visa application date or the date when they receive the biometrics and documents.

2) Another thing is When I had applied as a doctor in training in Dec 2018, Medical practitioners (Code 2211) were not under Shortage occupation list, but now they are. I am bit confused what to put in the dependants application regarding the same, as it will affect the fees paid.

I would be extremely grateful if someone can help with the same.

1. As long as you submit the application before their current visa expires, then it will be fine even if the rest of the formalities (biometrics, etc) are completed beyond the expiry date.

2. Not 100% sure. Wait for others to confirm. But does dependent's application fee change based on Shortage Occupation List?
My advice is based on personal research/experience, so pls check relevant UKVI sources.

Thanks for your reply.. I was worried, if the Visa approval date goes beyond the last date on BRP, technically they would have overstayed. Hope this won't be a problem.

Regarding the CoS, yea the fees for shortage occupation and non shortage one are different.
