MN1 Application form Help Needed!!!

Dear Forum members,
I have recently got ILR and my wife hold spouse visa.I will be applying for British citizenship next year. I am applying for registration for my 6 months old daughter who was born in the UK using the MN1 form on her behalf. She is the applicant but doesn't have an email address so i am using my email address. When filling the application form, it asks for the following details.

1. Registration email address
Who does this email belong to?
(Required)
-The applicant
-An immigration adviser based in the UK
-Someone else

what shall i write here please?

2. The other question is as follows,
Register child as British citizen
Are either of the parents of the child (as stated in this application) applying for naturalisation as a British citizen on an AN form?
Yes or NO.

What does this mean and considering our status above, is the answer yes or NO?

3. My daughter have just 'Birth Certificate' and hasn't got any passport or ID card etc. On the application form, it asks for the current child nationality. Do i need to write 'Indian'? Also it is asking for her passport and id document etc. what shall i do?

4. The application is asking for 2 referees, one referee need to know the child in professional capacity like GP, Health Visitor etc. The other referee can be any British citizen. The issue is 1st referee. I have discussed with GP, Health Visitor etc but none of them are able to provide their personal details. My daughter doesn't go to any nursery yet so what shall i do?
