If a 2 year visit visa is rejected, will they granted 6 months?

Hi, thanks in advance for your help. I have a few questions.

1) If someone applies for a visit visa just under 3 months in advance of intended date of travel, will the visa start from the date of issue or will it start from a time closer to intended date of travel?

2) Is it okay to apply for a visit visa before your previous visit visa expires?

3) If you have previously been granted a standard visit visa without issue, and then you apply for a 2 year visit visa but they reject you for this, are they likely to issue you with another standard visa instead or they will just issue you no visa at all? What I'm wondering is if it is more risky to apply for 2 year visa.

4) if you receive a new visit visa with a start date which is after your current valid visit visa ends, can you hold both visas or they will still cancel the old one?

From the time closer to the intended date of travel
2) Is it okay to apply for a visit visa before your previous visit visa expires?
Yes, unless you apply for the travel within validity of the old visa. Note, that it is likely that your current visa will be cancelled. I personally had this unpleasant surprise though my new visa didn't have the overlap with old one.

I was always told that if they don't want to issue longer visa, they will most likely just give you a shorter one. You'll lose difference in fees then. I don't know if there are any official rules available to public re this.
