MN1 form child under 5 online application

hi all,

I'd like to register my child who is under 5 years old as british citizen. I have an EU settled status. the online application asks for 5 years of address history for the applicant. Shall I provide my addresses? child has always lived with me since birth, but is only 1 years old.

also, child's healh visitor is refusing to act as a referee. is child's midwife a professional who can act as a referee?
