Help me : Non-EEA Family member want apply PR

Please any advice and help ...
I want to apply for non-EEA Permanent Resident after because Ill finish my 5 years .. my wife she got it and she has no British citizenship.. I tried only to got the right form but I didn’t find which one ?
Can anyone help me which is the right form for apply for “Permanent Resident for Non-EEA Family member ?

I tried online to got the right form to apply for PR as non EEA but I didn’t find which one ?
Can anyone help me which is the right form for apply for “Permanent Resident for Non-EEA Family member ?

Also apply under the EU Settlement Scheme. You should get "Settled" status under that scheme. But the EEA PR card allows visa free travel with your wife to EEA countries.
