Worked before joining EU - now just problems

I've worked legally in UK before Poland has joined EU [based on work permit]. Then continuously in the same place for 2 years.

It enabled me not to register with WRS.

But also my original WORK PERMIT has been lost by employer.

I'ev applied with EEA1 for Registration Certificate, explaining in the letter that I can not provide original WP [work permit] coz it has been lost. But provided copy and hoping the can obtain info from their files. They didn't. Sent me back.

So I've asked WP bureau for a reprint. Instead the sent me letter.
IN few words:
no reprints available, due to period of time that has elapsed they are unable to provide replacement documents. This letter should be used as "proof of confirmation that you are registered under this scheme" [WRS possibly].
letter contains reference number starting with "A8/".

Now I want to complete form EEA1 to obtain Registration Certificate, but I don't know what kind of details should I place there or what kind of info WRS hold about me.
Where to find out or where to start from ?

Do a search for Subject Access Request on the forum. You may tailor the SAR letter to specifically ask about your WP and WRS records.
