employment details query

Hi, i need help to fill Nuterilisation form.
I am currently a director in two companies since 2013,
how can i enter two current employment details?

also recently i have received my employement history from HMRC, in my history i can not see any details of my 1 company, this may be because i never run payroll or neither i get any income from this company, should i still mention this compnay in the form or should i enter only the details from employment history received from HMRC?
