IHS reimbursement

Dear all,

I would be grateful if you could advise on how to request a reimbursement of an IHS that has been paid twice.

I have paid 3 years IHS for a Tier 2 visa and after 1 year I switched to GTV and paid for 2 years coinciding with the last two years of my previous Tier 2 visa. Overall, I paid 2 years of IHS twice!
