ICT Dependent switch to Skilled Worker Dependent

Hi,

I am switching from ICT to Skilled worker and submitted my application for which biometric is not yet done.
I am filling the for for dependents application at a later date and got few doubts.

1) Can the dependent form include both my wife and kid (5 years) in a single form.
2) I got a question "Is Main applicant's sponsor agreed to certify your maintenance on their CoS? "...In the CoS, I can see "Tick to certify maintenance for migrant (and dependants, if applicable): Y "
Could you please help me what shall I select for the question asked above.
3) In document section , it asked "Confirm if you relying on evidence of funds available for your maintenance and accommodate provided by Main applicant in their application"
Option 1 - Yes - Main applicant has supplied evidence of funds for maintenance and accommodation.
Option 2 - No - I need to supply additional evidence of funds for maintenance and accommodation.

As I have not provided any Bank statement in my primary application, do I need to select Option - 2 and provide Bank statement?

Please help me with all the above queries.
