Referee declaration questions

Hello,
I wanted to ask a question. I see that this was posted as the referee delcaraion form: https://visas-immigration.service.gov.u ... on_MN1.pdf

However, I used pages 20-23 on the AN form (https://assets.publishing.service.gov.u ... -01-21.pdf).

Would this be an issue?
