Parent Consent for Child British Passport

Hi ,
I am applying for my child's first British passport , the following mentioned in the document checklist
has any one got the parent consent template format , if so could you please share the same

Consent letter :-
If you don’t have parental responsibility, also send the following:
a signed and dated letter from the mother (or other person with parental responsibility) giving consent for the issue of the child’s new passport
