spouse visa in UK

Hi i have ILR and my wife was Tier 1 dependent for 3 years after she changed to 3 years refugee and now she wants to apply for spouse visa from inside the UK.

Will she be able to accumulate the above years and apply for the 10 year route?

What requirement for her apply?

How long does it take from inside the UK?

Is there premium service for spouse visa application?
