I submitted my naturalisation application last week and the form was there among the compulsory documents. It might be the case that they're making this form compulsory now but I don't think it's the case for past applications. I wouldn't worry too much. I'm also not depending on anyone and don't have any co-signers.
its been 4 months now and I've received no email that my application is being processed or that they need more stuff. Now im worried that maybe i need to send this consent form?
I sent HO and email today so will let you know what they say.

Many applicants are saying that this form was not there. That might be the case but I can't recall.
I did send a signed copy to FurtherNationalityenquiries@homeoffice.gov.uk yesterday.
Will update this post if I get a response.
