PBS Dependent Visa for newborn child in UK

Hello,

I am applying for PBS Dependent visa for my newborn child. Since I am the main visa holder and applying the form on her behalf, I have a couple of queries:

• I was initially on TIER 2 (General) Leave to Enter visa type for more than 3 years. Now since almost half of the visa duration is already passed, shall I just mention COS length of less than 3 years?

• Whose details should I fill under Third Party Declaration considering I am filling the form myself with no help from an immigration advisor? My COS specifies to certify maintenance for my dependents(if applicable) so I am not attaching any bank statements with the form
