continuity in 10 year ILR

For ILR based on 10 years long term residence, it is said 'You should continue to obtain extensions to your status in the UK throughout the ten year period' and that this is especially relevant to students; but are visas obtained at the airport in the UK considered to be within the UK or do they break continuity (ie, if jsut before a visa ended a very short trip was made with the visa stamped in the passport at the airport)?
