Skilled worker child dependent Visa - parent consent letter

Hello Everyone,

I'm looking out for the help on the parent consent letter format.

I'm applying for the dependent skilled worker visa along with my 2 year old from outside UK and my husband is already in UK. I filled in the application form for my Son and in the final application checklist I got a list in which below is one of items in the list.

A letter of consent that confirms:
• the relationship between you and your parent(s)/legal guardian(s)
• consent from your parent(s)/legal guardian(s) to the application
• consent from your parent(s)/legal guardian(s) to the living arrangements in the United Kingdom and the address where you will be staying
• whether one or both of your parent(s)/legal guardian(s) have responsibility for you
• Both parents/legal guardians must sign the letter, except where only one parent/legal guardian has sole responsibility for you


Could some one please help me with an example letter.
