Details of previous medical treatment in the UK

Hello,

I currently have a Tier-2 visa and I want to apply for the Skilled Worker visa(change of Employer) now.
But I have two questions:
1. I used my BRP as the document for applying and I used the ID-Check app with my BRP. The application form also asked me for the National ID Card number and I provided my National ID Card number but it did not ask me any questions about my Passport(passport no, expiry date, etc.). I just want to make sure that I have done everything correctly?
Because my national ID card is not an international card and it is not in English.

2. My wife has a PBS Dependant visa and we want to apply for visa for her immediately after I get the visa(decision email) from the HO. I am wondering how should I respond the below question?
"Are your partner or children applying with you?"

Hello,

It appears that the medical treatments/appointments in the UK must be declared on the visa application form. My question is: do I need to also declare my dental treatments? Also, what evidence I can provide to show that I have paid for my dental treatments? I do not have any receipt for my payments.
