Spouse Visa Extension Declaration Form

Hi Guys,

Hope everyone is keeping well.
I am in a process of submitting documents for spouse visa extension from the UK.
I am bit confused on consent verification form as attached.

For part 2 and part 3, do we both, the spouse and applicant need to sign or just the sponsor needs to sign it.

We do not have any joint bank accounts or joint named corresponding letters.

I am unable to attach part 1,2 and 3 on this post.

Any help is appreciated.

Applicant to sign Part 1, Sponsor to sign Part 2 and if no 3rd party support write Not Applicable in Part 3( don't leave it blank).
