Supporting document clarification for ILR

For ILR Set(O) via ancestry, supporting documents:

I have a courtesy Dr title (dental) which is on my malpractice insurance for this upcoming year that I intend to use for Document(s) showing that you are able to work and intend to take or seek employment in the UK.
This is also on some of my supporting utilities statements which I will be using for the upcoming naturalisation application (hopefully) later anyway. 
I thought it would be overkill to add a title onto are you known by any other names but should I still address this other title in a cover letter if I am using documents with it on? 

I have applied in my maiden name as the name I usually go by because I do not intend on using my married name in the UK until such a time as I need to renew my SA passport (in maiden name).
On my application I answered that I am known by another name through marriage & that I have not entered the UK using this name. 
I did this to be transparent & also as once I am through my settlement & citizenship I intend to start my very patient SA partner's spouse visa application.
On supporting documents they list: 
Evidence that you have a grandparent who was born in the UK or Islands (if they were born there before 31 March 1922) such as: The birth certificate of your grandparent who was born in the UK or Islands ect.
If you, your parent, or grandparent have changed name since birth, evidence of this, for example, marriage or civil partnership certificates.
Should I attach a marriage certificate as proof of this name and change of marital status which I declared in the application?

A title is obviously not part of your name. It is perfectly fine if a title like 'Dr' appears on a letter or document. That does not mean your official name is different. Your name normally must match what is in your passport for any immigration or nationality application. If you do not intend to apply for a visa in your married name, you should NOT choose that option on the form. The form is not a full disclosure, the answers are the form are simply used to create the document list at the end and to guide the caseworker. In the UK, you may use your married name without any legal process like a deed poll. No need to declare this on the from or select that option.

For nationality applications, things can get complicated if you use other names. Your name must match the name on your passport and naturalisation certificate or else HMPO will refuse to issue you a passport. You have to be careful around that
