Appendix and Consent forms

I would appreciate some help please
Have applied for spouse visa for my cousin
However I remember in my application a few years ago there were consent form 1,2,3 that required to be filled out as well as a separate appendix form I think it was called.
This time around we haven’t been prompted to fill out anything .. are they no longer required ?

The appendix form is now incorporated into the online form and therefore does not need being filled separately.

Till few months ago they are required and you still get to fill the consent forms; sure you didn't see link to download form as part of action when you submitted online form?

Yes I assisted her in applying.

It’s not on the checklist and there is no link within the online application
