I am applying bno visa extension. When I answer Yes to question "Have you ever received medical treatment in the UK?", the next question is "Were you told that you had to pay the hospital, clinic or doctor's surgery for your medical treatment?"

This question is very confusing to me. I have paid the IHS when applying the visa, so no need to pay for any medical treatment in NHS. But I have received a medical treatment in a private hospital, that of course need to pay. Everyone knows that private hospital need to pay.

Should I answer Yes or No?

It is focused on NHS treatment although to be fair it is not absolutely clear. Because you are not required to pay for NHS treatment by paying IHS then the answer is NO
