There's no such certificate - they tick the relevant boxes in the form.
3. What to do if School & GP denied to give their personal details as a first referee?
Find other referees who qualify - UKVI are aware of the problem.
4. Do they need to write life in UK/English qualify exam?
Not required for under 18s.
5. Do they need to attend ceremony?
Ceremony is for over 18s only.

Note for nationality applications there are very few different forms, for adults and children it’s just one each, AN and MN1.

Can you please explain what category you have selected? Under what heading is "register as a child"? By "Other" you mean "at Home Secretary's discretion"? Were children born abroad?
Please clarify.
Caseworkers will know under what category your children qualify based on the documents submitted, if they qualify for registration in the first place. It's not a case of you choosing the category and being refused because you ticked the incorrect option.
I don't believe you need to do anything - caseworkers would have contacted you by now if there's an issue.

I would doubt that it would be straight refusal - especially if children qualify per the documents submitted.
