Health and care visa extenstion

Hi
This about health and care visa extention for dependents. I am doctor on health and care visa

I am applying for visa extension for my family from within UK.
They already have stayed in the UK for about 3.5 years. They need 1.5 year stay before applying for ILR.

i completed the extension application but in the end it is generating fees of 551 (5 year visa ) rather than 284 (3 year fees).

It doesnt ask anywhere in the application if i want to apply for 3 years or 5 years
Only thing i could think of is the question where it asks how much was the main applicant COS for
3 year or less
Or
3 year or more.

I selected 3 year or more as COS was for 5 year (training COS)

Do we need to selected 3 year or less in that section to generate 3 year fees ?
If anyone in the same boat ?
Do we need to pay 551 ?
