Home office consent form to verify application information

Hi,

I am submitting the visa application for my newborn baby born in UK as dependent on my skilled worker visa. At the end of the application, they have given me a consent form for home to verify application information. My query is do I sign this application myself and write my name under applicant name or do I fill my baby's name and take his thumb impression? I am really confused.

You are already applying on baby's behalf as parent or guardian.
