OCI APPLICATION

Hi, I am applying for OCI's for my children and I am at the questions which asked has any over family member applied for an OCI. I have in the past and at the same time will be applying for a surname change on my OCI. Here do I write the previous surname I had in which I made the application or do I need to write my new surname and note it has now changed?
