NICOP/Visa questions and using the Pak Identity app

I hope you are well.

I am applying for a NICOP for our child.

I am a British born Pakistani who is married to a Pakistani born British national who now has her UK passport (through the spouse route). I have a NICOP, she has a Pakistani passport and NIC. Our cards are not updated to say we are married. We have 2 children, one aged 6 years, the other 1 and half years old. The 6 year old had a NICOP made through her mothers card (from what I recall about 4 year ago at one of the offices). My wife is using her unmarried name on her passports and ID card. On the child's birth certificate (and other documents) it is her married name. We have a marriage certificate. We are looking to make a NICOP for the 1 and half year old using the app. I have some questions I hope you can help with please:

1/ When creating a new NICOP for our child under the "relative details" section it asks for a citizen number so I have added mine in there. The "relation with applicant" only gives options relating to children. I'm a bit confused as the section before "family details" under head details says "relation with head". If I put relation with head as "child" I have to put "child" under "relation with applicant" also. Does that make sense? To me it doesn't as the relation to the applicant (who is the kid) should be parent?

2/ Will either of me and wife not updating marital status cause any issues?

3/ If we apply for the NICOP and there are issues can we still apply and get a visa for our child in case we want to book travel and the NICOPs not came?

4/ If we apply for a visa will anything that I've said cause any issues?

If you have any further advice/information I would be grateful if you can share this please.

We would be looking to travel mid March.
