OCI Application query

Hi,

There is below question in OCI application.
"Have you ever changed your name? If yes, click the box and give details."

applicant last name is changed after marriage.

1) does above tick box to be selected as "Yes" ?
2) Or no selection required as name not changed and only last name is changed ??.

note: naturalisation certificate mentions full name as name after marriage. passport also have name after marriage.
also mentions name at birth which is with applicants before marriage last name.

Hi,

I have applied for OCI and application and is in progress.

planning to apply for my wife's OCI. there is below question in OCI application form.

"Have you/any member of your family applied for Overseas Citizen of India registration earlier."

1) what should be the response to the question.
2) This question has option as granted / not granted and cant select granted yet.
