HSMP application - from inside or outside UK?

Can a person who is in the UK on a visa category which doesnt permit switching into HSMP visa apply for HSMP from UK. But after getting the decision letter, can she/he travel to their respective home country and obtain Entry Clearance to UK ?
I heard that the decision letter states both clauses...if you are in the UK, apply for FLR...and if immigration rules doesnt permit switching, apply for EC at the British consulate. Can somebody who has got the decision letter recently throw some light on this please.
