Correct application forms for me and my dependent ???

Correct application forms for me and my dependent ???

Hi

Im going to fill the application form for Tier1 in March first week . Yesterday i took print out the application forms for me and my wife(dependent).

Are these right application forms . Im going to fill now .

1) POINTS BASED SYSTEM FORM(VAF9 NOV 2008) PERSONAL DETAILS
2) POINTS BASED SYSTEM FORM(VAF9 AUG 2008) APPENDIX 1: TIER 1
(GENERAL) MIGRANT SELF-ASSESSMENT

3) POINTS BASED SYSTEM FORM(VAF10 NOV 2008) DEPENDANT FORM


Are these correct form for me and my dependent to file tier-1 application form or do i need to fill more forms.

Please let me know
