There is not fixed format. You just need to write letter, inviting her. Explaining about financial arrangement (who will pay for her travel, and where she will live).
Documents needed are employment documents, 3-6 months salary slips and bank statements, utility bills(if applied), tenacy agreement. If she does have some saving in her account, you can show that,too.
In short, you need to demonstrate that , you are(or she is) finanancially capable of her stay here .
And form needed to fill for this visa does need requiered information to fill in.
