Urgent !!! Tier 1 Issue..

Hi Gurus,

Am getting the application from the link http://www.ukvisas.gov.uk/en/howtoapply/vafs/ and the name of the form am using are as follows:
1) Application form VAF9 - PBS Migrant
2) PBS Appendix 1 General Migrant self assessment form

Pls confirm if am good to go with the form that i mentioned above, are these the correct forms am looking at.

As the pdf forms are editable, can i fill it on my computer and then take print out of it and submit the same.
Is blank and white print out okey, or should i take a color print out of the forms.

3) Strange but my parents dont have their date of births. what should i put in the application form.
Pls help........!!!!!!!!!!!!!!!!

Editable pdf forms are okay as long as you sign the printout manually. I confirmed this with HO, and this is what they told me.
