Dependant Visa for HSMP applicant

Hi,

I got HSMP visa on 30-Nov-07 and my daughter is born last month in UK, now i want to apply for dependant visa for her.

Do i need to use FLR(O) or PBS Dependant form?

also what are the basic documents i need to submit?
