tier 1 extension - pls help

hello

i came here last may under hsmp visa ....it expires in feb 2010 , however i wish to apply for extension under tier 1 in september 2009 itself as i need to travel later in the year.

Please can someone give me the link to the latest form and guidance notes , i cant seem to find the relevant one

also , is this extension under tier 1 that i need to do or switching from hsmp to tier 1
