PL HELP (URGENT )Extension appl returned due to non payment

The student extension application was made in country on 30th may, just 1 day before the visa expired (on 31 May).

The appl and passport was returned as there was a problem with the debit card payment.

What should be done now.

The form version has changed since then,

Should I use the same form they sent back, or should I fill in the new version.

Is there any thing else that I should keep in mind.

If the form is changed then you need to use the new version of the application form as it will be considered as new application.

And as your visa is expired now so you need to write a covering letter explaining everything about your previous application.
