Student Depenant application from India

I am on a student visa (NOT TIER 4) under the rules in place before March 2009.

My wife is in India and i wish to apply for her dependant visa.

Do i need to fill out the Tier 4 depenant form?

What documents do I need?

Is there any maintenance fund requirement?

Please help.
