doubts on filiing appendix 5,vaf 9

hi, i have difficulty in filling appendix 5.
1)question 5.4 says regarding payslips or personal bank statements, or building society /savings account pass book for six months, as proof of working from the sponsor. as iam working in india and iam applying for tier 2 general. is it applicable to me.

2)question 9.3 on maintenance asks for a letter from A-rated sponsor with regard toaccomadtion ,for the first month.does a faxed or emailed document enough.and can a individual be an a rated sponsor.
IN VAF 9 form it asks for date of travel. 3)can i give approximate date
Iam applying for my wifes visa after i get the visa is it all right to mention no to the question that asks whether my partner will be joining me later

Are you applying for the Tier2(G) Visa?

Did you get the Certificate Of Sponsership letter and Unique Identification Number (UIN) to fill in the application??

216. The evidence of the sponsor certifying
the maintenance must be a letter from the
A-rated sponsor. The letter, which can be
posted, faxed or scanned and e-mailed to the
applicant, must be on official letter-headed
paper or stationery of the organisation, be
signed by a senior official and must show:
• the applicant’s name;
• the applicant’s certificate of sponsorship
reference number;
• the sponsor’s name and logo;
• confirmation that the sponsor has certified
the maintenance; and
• details of limiting the undertaking

HO allows date anywhere in a range of 3months from the date you apply..

Yes if ur spouse will join u after 12months u can say NO
