Work experience question

To apply for HSMP, and what documents to you need to prove your work experience ?
Do you need salary letters from all your jobs back to 5 years back ? (I am over 28 years old and have been working in UK for the last 2 years)

You need letters from all employers for all jobs going as far back as you are claiming. Therefore, if you are claiming 5 years experience, you need to get letters from all employers in this 5 years time period.

Is this a reference letter that says something like that you work there from date x to date y , and states your job duties ?
Do I need a salary letter from any job longer than 1 year ago ?

I have another question.
I have worked at 3 places, say company A, B and C in that order.
Since my supervisor does not work at company B anymore, it might be difficult to get reference letter from company B.
Assuming, the total number of years I spent at company A and C are 5 years, would it be acceptable to give reference from only company A and C (and not from B) ?

Sorry, I have another question.
If I need to get reference from company B, would it be acceptable for the reference letter to come from my supervisor there ?
Another problem is my supervisor has moved to another company, would it be possible for him to write the reference letter using his current company letter head ?
