Need help! confusion in dependent form

Hi,

I am in UK on HSMP Visa from 01/01/2008 to 01/01/2010. I have got the approval letter from home office on 10 October 2007. I am applying for Tier 1(G) extension in October 09 for me and my dependent.
• I am confused if the initial entry approval letter is valid for my Tier 1 (G) extension.
• If it is valid then what is Tier 1(G) extension fee: is it 820 or 400 GBP for the main applicant?
Under Dependents form maintenance question
• Under maintenance section Question J4 about Letter from A-rated sponsor…. Do I need to answer this? I think it is for students. If yes, than what kind of letter do I need to attach?
• Do I need to answer questions from J5 to J16? I think it is not for Tier 1 dependent extension. Please confirm

You have already used your initial entry approval letter to get the HSMP visa. Your application will be a fresh application and the fee of £820 for main applicant will apply.

The form for Tier 1 General and students is completely different. There is no way a question for student visa will appear on Tier 1 G form.

Questions starting with J are for Age Assessment, for people who are applying fresh for Tier 1 (G), i.e. non-extension.
