Immigration History section with lost passport? Please help!

Hi, the Home Office / Royal Mail lost my passport I originally entered the country with in 2003 (studying English - I then came back after 6 months in 2004 to study again) as a student during a student visa renewal. I since got a new passport and student visa which runs out at the end of this month. Can anyone advise if this is an ok way to fill in section D of the PSW, as I do not have any record of my original entry, just a rough idea of the dates?
D3. What date was the entry clearance or visa issued?
My answer: "10/03/2004 (best guess)"
D4. Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document.
My answer : "UNKNOWN (SEE C7)"
This refers to an included Home Office letter acknowledging the passport had been lost. I presume they want the original entry visa reference and not the current one listed in my current passport which was issued within the UK?
D10. When did the applicant arrive in the United Kingdom?
My answer: "10/03/2004 (best guess)"

I also put "not known" under ticket booking reference.

Also, one other question: I didn't report the passport stolen, as it was lost in the post not stolen as far as I know. Is it advisable to now report it to the police for application purposes?
