Question about form SET(O)

On page 5 of form SET(O) there are two fields:
1.4 Your full name as in your passport or travel document
and
1.5 Surname or family name

How one needs to fill them?
It would appear that the logical way to fill this is:
1.4 John Smith
1.5 Smith
but are they really asking to duplicate information?

May be it should be:
1.4 John
1.5 Smith
??

Full name (as in your passport or travel document) should include surname.

Please do not over complicate something like this. If they ask for duplicate information just write it down. It won't do any harm.

I totally understand. I think everyone develops OCD when filling out any visa applications
