Dependant application form Query - A4 to A6

Hi,
I am applying for Tier 1 and also submitting my wifes dependant application along with mine. I have a query on the DEPENDANT form.

a) A4 asks for Main Applicants name.
From the the instructions on Page 4 related to this, am I right in interpreting that this need not be filled in as I am putting her application with mine.

b) A5 asks for main applicant date of birth.
If the above is true then do I need to skip this as well?
The instructions on Page 4 do not qualify when this should or should not be filled in or link this with A4 so I am unsure.
