In UK on Work Permit, got COS from new employer - what now?

The subject says it all really.
Been in uk on a work permit and have just got a Tier2general COS from the job i want to move to. what do i do now?
what visa form do i need to fill in
how much is it
any other useful advice would be greatly appreciate
