ILR SET(M) PEO

I am about to apply for ilr set(m) at peo this month.my wife(british citizen) is currently using my last name on all her documents from work,banks,utility bills etc but she has not change her last name on her passport to mine yet.

my question is,on the set(m) form,what last name should i use when filling her name?is it her last name(surename) on her passport or my last name that she is using on all her other documents other than her passport.

please reply if any one can help.
