Hello Folks,

We recently put observation stamp on my wife’s passport (2nd page), it states following:
Consequences of marriage name added as: Rupali Jaiswal
But front page and last page of the passport still display her old name as Rupali Jain
Now here I am bit confused, should I use Rupali Jaiswal on the dependant form or Rupali Jain, could you please help me out?
